Sundial Mind Care is a self-pay practice; we do not bill or participate with insurance plans.
Payment is due at the time of service.

🔆Medication Costs
Office visit fees do not include the cost of prescription medications. During your appointment, we are happy to consider your prescription insurance coverage and medication costs when selecting appropriate treatment options.
🔆Superbills
Upon request, we are happy to provide a superbill that you may submit to your insurance company for possible out-of-network reimbursement.
Reimbursement is not guaranteed and depends on your individual insurance plan.
🔆Missed Appointments & Late Cancellations
Appointments cancelled with less than 24 hours’ notice or missed without notice may be subject to a $50 cancellation fee each occurrence starting with the second.
Repeated missed appointments may result in discharge from the practice.
